LOCATION:\u00a0<\/strong>Porter Avenue Office<\/p>\r\n

IMMEDIATE SUPERVISOR:<\/strong>\u00a0Housing Program Manager<\/p>\r\n

HOURS:<\/strong> : Full-Time 40-hour work week Monday \u2013 Friday, 8:00 am \u2013 5:00 pm, with the exception of one day of the week being 10:00 am-7:00 pm. some evening and weekend work may be required.<\/p>\t \r\n

SUMMARY:<\/strong> Responsible for oversight of Rapid Rehousing programs funded by the Department of Housing and Urban Development. Provides case management services and transportation to victims of domestic violence, dating violence, stalking and human trafficking who are enrolled or eligible for enrollment in this rapid rehousing programs. Ensures compliance of grant\/funding requirements.\t \r\n <\/p>\t \r\n

POSITION REQUIREMENTS:<\/strong> Essential position requirements include but are not limited to: \t \r\n Minimum of a bachelor\u2019s degree in social work, psychology or related field. Two (2) years of related experience preferred.<\/p>\t \r\n

Must be able to work effectively with diverse groups of clients, staff and individuals in the community.<\/p>\t \r\n

Ability to work diplomatically in order to gain and maintain professional relationships with various community partners and contacts<\/p>\t \r\n

Ability to handle multiple projects simultaneously; work both independently and in a team environment<\/p>\t \r\n

Ability to express complex information and ideas both verbally and in writing.<\/p>\t \r\n

Ability to work within agency mission, philosophy and policies\/procedures. <\/p>\t \r\n

Must have a reliable vehicle and meet agency driving policy requirements, including a valid driver license and proof of required automobile insurance. Ability to obtain Class D driver license. Local travel required<\/p>\t \r\n

General physical requirements include but are not limited to: bending, stooping, climbing stairs, prolonged sitting of more than 4 hours. <\/p>\t \r\n

ESSENTIAL DUTIES INCLUDE BUT ARE NOT LIMITED TO:<\/h2>\r\n

Program<\/h3>\r\n
    \t \r\n
  • Ensure the implementation of program goals and objectives as specified in the grants<\/li>\t \r\n
  • Develop and maintain working relationships with landlords and community partners<\/li>\t \r\n
  • Complete all documentation requirements for housing programs<\/li>\t \r\n <\/ul>\t\r\n

    Clinical<\/h3>\r\n
      \t \r\n
    • Conduct prescreening assessments with candidates to determine eligibility for housing programs.<\/li> \r\n
    • Facilitate client development of social supports, safety plans and service plans<\/li>\t \r\n
    • Collaborate with other community partners and social service agencies to seek and obtain services for clients<\/li>\t \r\n
    • Conduct home visits with program participants<\/li>\t \r\n
    • Provide recommendations to clinical staff for housing programs<\/li>\t \r\n
    • Provide client transportation as needed to community locations<\/li>\t \r\n
    • Attend and participate in weekly clinical and housing staffing meetings <\/li>\t \r\n <\/ul>\t\r\n

      Agency Compliance<\/h3>\r\n
        \t \r\n
      • Maintain comprehensive and organized files utilizing the automated data and case management system<\/li>\t \r\n
      • Adhere to the agency\u2019s confidentiality policies, maintaining a high level standard of confidentiality<\/li>\t \r\n
      • Compile and accurately report all applicable program statistics and educational activities on time and as directed <\/li>\t \r\n
      • Report any known or suspected child abuse to immediate supervisor<\/li>\t \r\n
      • Report any suspicious activity or persons<\/li>\t \r\n
      • Attend and participate in agency staff meetings and trainings <\/li>\t \r\n
      • Report to work on time as scheduled<\/li>\t \r\n
      • Maintain a valid driver license and proof of required automobile insurance<\/li>\t \r\n
      • Perform all other duties and\/or participate in special projects as assigned by direct supervisor or Chief Executive Officer.<\/li>\t \r\n <\/ul>\t \t\r\n

        Other Duties<\/h3>\r\n
          \t \r\n
        • Participate in hurricane evacuation and facility preparation.<\/li>\t \r\n
        • In case of emergencies and extreme circumstances, will be required to assist with 24-hour shelter coverage.<\/li>\t \r\n
